I don't care what protocol it uses, I just want the middle button to paste
in X :-)

It can't be any of those things. Straight out of the box all three buttons
work fine - with gpm. It's when I go into XFree86 that I can't find any
configuration that works.

SInce I've had one report that it works, I'll try gpm's relay mode and
hope that gpm can translate whatever wierdness there is to something that
Xfree86 understands.
