You can get your mail to run a program either via your .forward file, alias
or virtusertable (I think).

Such as

"|/usr/bin/mail-monitor.pl", realaddress

If you have a sound card you can then get the script to cat a sound file to
your sound card. You could even get the script to parse the headers and play
different or no sound on different types of messages. For example some
mailing lists you may ignore, for SLUG you might play an exciting beep and
for personal mail direct to you you may play a message "You've got mail". 

You could also use some speach to text software to announce it eg "A new
mail from Rodos Re: SLUG ding beep dong etc". Would be easy to do.
